Order Form for Electronic and Print Publications

The form on this page may be used to order electronic or printed publications. When received, I will send a PayPal invoice for the items desired. The invoice will guide you through the payment process - you do not need to have a PayPal account to use this payment method. If preferred, the form may also be printed and sent with a check to the address on the form. Please make checks payable to DiverseArts, LLC.

For electronic versions, I will email (or mail) a link to the location of the download file along with the password upon receipt of payment. Each document has a reference number for tracking purposes. Files should be saved to your local drive within 2 weeks of purchase. The files may be printed.

Please respect the copyright holder's rights and do not redistribute any of the downloaded files. They are for the use of the original purchaser only.

In the event that you lose your password, please email me. I maintain a record of all purchasers and corresponding reference numbers and passwords.
